The idea is that toothwear is a surface change that progresses slowly, and radiographs aren’t useful for tracking it. The pulp–dentin system often defends itself during slow wear, so even when wear is severe, about 99% of teeth don’t have apical pathology. Radiographs are intended to detect internal disease, not hidden surface wear, so taking them periodically to monitor wear would expose the patient to radiation without changing management. That’s why periodic radiographic follow-ups aren’t recommended for toothwear.
